Issue : 
Ticket creation form is not loading.

Steps :
1. Log in to the system.
2. Navigate to "Ticket" module.
3. Click on "+" icon at the right bottom of the page. 

Expected Result :
The user should be able to view ticket creation form.

Current Result :
The ticket creation form is not loading.
