1 - Log in to app using valid logion credentials.
2 - Successfully navigate to "Customers" module.
3 - Click "+" icon to navigate "Contact Creation" form.

Expected result
  >> User should be able to select "Customer Segment"
  >> User should be able to enter "Joint Client Code"

Current result
  >> "Customer Segment" & "Joint Client Code" fields are not added to contact 
     creation form

>> User should be able to add "Joint client code" & "Customer segment" when 
   creating a customer.
