Error message does not clear

Steps to Recreate:

Login to the system with valid credentials
Navigate to the above URL(CONTACT BASE SELECTION page)
Click on 'Create New Contact Base' button
Try to fill the fields of 'Create New Contact Base' without giving a value to 'Description' (Error message will display)
Close the modal
Re-open the modal


Expected Result:

Error message should disappear


Actual Result:

Error message does not clear
